To request authorization for an item return, please complete this form, providing as much detail as possible to expedite processing. 

Please note: Warranties may be voided if equipment damage is evident, even if the unit is under warranty. 

For non-warranty items, a service or inspection fee may apply. The sender is responsible for freight costs to our warehouse; we will cover the return shipping.

Attach the invoice for the item (PDF or photo, up to 3 files, 10 MB each). Warranty claims are assessed against proof of purchase.
Not a warranty job, so tell us how far to go before we contact you.
If the repair will cost more than this, we will contact you for further instructions.
